Ordering Serial Replacements

January 1996

I.    General

A.   When a serial that has been received is missing, or a publisher refuses to honor a claim for a missed issue and the issue or issues are still wanted for the collection a Serials Replacement request may be sent to the Serials Replacement Clerk in Serials Acquisitions for replacement action.

B.   Acquisitions will then send the request in turn to USBE, the publisher, and appropriate dealer specializing in replacement materials. The cost for the publications is charged to the individual subject monograph fund.

II.   Serial Replacement Request Forms

The serial replacement request forms should be filled out in full by the requestor as follows. Acquisitions prefers that the forms are typed. There are five copies of the form. The first copy is the requestor copy. After the form is completed, the requestor should tear off and retain this copy for his/her records. The other four copies should be sent to the Serials Replacement Clerk in Serials Acquisitions to generate the order.

A copy of the serial replacement request is shown below and the numbered boxes correspond to the numbered descriptions that follow. The boxes must be completed by the requesting unit.

1.   Serial entry - The complete serial entry is typed in this box.

2.   Vol., no., date - Volume and/or issue numbers and dates for those items should be indicated here.

3.   Publisher's address (from most recent issue) - Complete publisher's address should be included.

4.   Call number - Supply the full call number.

5.   Destination - Fill in the destination to which the issues should be sent.

6.   Requested by - Should be initialed by the requestor.

7.   Approved by - Should be initialed by the appropriate subject bibliographer.

8.   Fund - Correct monograph subject fund is put here.
