Injuries to Employees During Working Hours
Employee
Employees who suffer
a work-related injury should immediately report the injury to the supervisor
Supervisor
1) If the injury/illness is critical,
supervisors are to immediately call an ambulance (911). The ambulance will
transport the individual to the nearest medical facility available for
treatment. The supervisor should then contact the Human Resources Office and
report the incident.
2) When the injury/illness is not critical, the
supervisor will ask the Human Resources staff to complete the Authorization to
Invoice MSU and direct the employee to report to the appropriate medical
facility designated on it.

3) For on-campus work-related injuries where
minor medical care is needed and the supervisor cannot drive the employee to
the medical facility, the supervisor can call the Spartan Cab.
4) Within 24 hours after the injury, the Report
of Claimed Occupational Injury or Illness must be completed. The Human
Resources staff will complete as much of the form as possible and forward it to
the supervisor and employee to finish. The supervisor and employee must sign
the form and return it to the Human Resources Office as soon as possible.
5) Return to work authorizations, with or
without restrictions, must be provided by the employee to his/her immediate
supervisor. Employees capable of performing the essential functions of their
job will be returned to work. Employees requesting reasonable accommodation of
their position will also be returned to work upon accommodation. Accommodation
requests should be communicated to the immediate supervisor. Supervisors
needing assistance with these requests should work closely with the Human
Resources Office and Coordinator for Handicapper Programs and Services.
Recording Time
Missed
1) Employees scheduled for doctor's appointments
and physical therapy for a work related injury will record time missed as
Release time. This time is also counted as FMLA leave and should be marked with
an asterisk (*).
Part time employees
are encouraged to schedule these appointments during the times they are not at
work.
2) For extensive absence, when the employee will
miss at least one full day of work beyond the date of the reported
illness/injury, the Human Resources office submits an Injury Absence Report
Form to the Worker's Compensation Office. Payment for this time is not to be
deducted from sick leave, vacation or personal leave accruals on the employees
Hours Worked Record. The Workers' Compensation Office will make any necessary
adjustments to the employees FMLA and leave accruals. Information
