The back has the centered circular logo of the Northwestern Consolidated Milling Company. The back cover has a stamp reading "The Northwestern Consolidated Milling Co. 115 East Market Street, Wilkes-Barre, Pa." The book lacks a serial number. 42 pages plus index.
