The front cover begins at the top with the title "The Ladies' Note=Book And Calendar" and continues with an illustration of a mother writing a letter while her son and daughter read. The father and little girl are watching from the right side of the table. At the bottom, "Compliments of World's Dispensary Medical Association, Buffalo, N.Y." is stated in black lettering. The back cover features a drawing containing a six story building which houses the manufacturer of Dr. Pierce's Standard Medicines. Below, there are two paragraphs titled "Consistency, Thou Art A Jewel" and "Do Likewise." The front end page begins with the heading "Beware of Swindlers" and is followed by an informational paragraph titled "Common Sense vs. Prejudice." The inside pages are dedicated to various receipts and "memoranda" pages. The receipts listed fall under the categories of soup, fish, cake, icing, pudding, and meats. All receipts entail the ingredients needed plus directions on how to prepare the dish. The "memoranda" pages are made up of blank lines for the purchaser to write down memos. The back end page is titled "Dr. Pierce's Compound Extract of Smart-Weed or Water Pepper" and contains promotions for various medicines offered by Dr. Pierce.
