Description:
The front cover has a light blue rectangle with a drawing of a blank book and a tiny child writing in it with an oversized quill and ink bottle. It has a burnt orange background. The back cover has the same background with a light blue circle and shows a little boy pulling a toy accompanied by a dog. Each page in the pamphlet has space to write in information about a baby. Manuscript name is across the top of the front cover. No page numbers.
